This data collection system has been created by the 'Analytical Expertise Center' public legal entity of the Ministry of Health of the Republic of Azerbaijan (hereinafter referred to as the Authority) to collect information from patients, patient relatives, and healthcare professionals about adverse drug reactions occurring during the use of medicinal products and vaccines.
The system's foundation was prepared by the World Health Organization's Uppsala Monitoring Centre, and the information you provide here will be transmitted to the World Health Organization's global database on adverse drug reactions. The provided information will be processed by the Authority to identify adverse drug reactions and ensure the safe use of medicinal products. At the same time, the Authority may need to contact you regarding the information you provide, so please provide your contact details fully and completely. The Authority ensures the confidentiality of the personal information you provide based on the requirements of subparagraph 19.2 of the 'Regulations of the Pharmacovigilance of Medicinal Products' (The Authority takes measures to protect the confidentiality of information about the patient or medical and pharmaceutical personnel related to the reports sent to the Authority, and this information is not disclosed without the consent of these persons, except for the cases provided by the Law).
